About this affidavit template
An affidavit is a written statement you swear or affirm to be true in front of an authorised officer, such as an oath commissioner or notary public. Offices ask for one to correct a name or date of birth, to report a lost document, to confirm your income or marital status, and for many other records. The format is always similar: who you are, numbered statements of fact starting with "That…", your signature as the deponent, and a verification paragraph, followed by the officer's attestation.
How to make an affidavit
- 1
Download the Word file (or edit online) and write the purpose in the subtitle.
- 2
Fill in your full name, parent or husband's name, ID number and address.
- 3
Write each fact as its own numbered "That…" statement, short and specific.
- 4
Print it on stamp paper if the office requires it (ask which value), or on plain paper if accepted.
- 5
Sign it in front of the oath commissioner or notary, who verifies and stamps it.
Tips for a valid affidavit
Ask the office first
Each office has its own rules on stamp paper value and who must attest it. Check before you print.
Sign in person
Don't sign in advance. The deponent signs in front of the attesting officer.
One fact per point
Short, numbered statements are easier to verify and harder to misread.
Never state anything false
A false affidavit is a criminal offence (perjury) in most countries.
